Venture Capital: Monogram Health Lands $375M Funding Round For Specialized At-Home Renal Care

Monogram’s aggregate funding is now $555 million.

Monogram Health, a company that provides in-home care and benefit management services for patients living with chronic conditions, has closed a $375 million funding round.

The funding will be used to support the expanded deployment of Monogram’s in-home kidney and chronic care delivery model, as well as its technology-driven array of clinical services. This model has been shown to improve patient outcomes and reduce medical costs, while also increasing access and affordability for patients with chronic kidney disease and comorbid metabolic disorders. Over 37 million adult Americans suffer from chronic kidney disease. More than half a million Medicare patients are on regular dialysis due to end stage renal disease (ESRD). These patients can take advantage of Monogram’s services which include complex case and disease management, utilization management, and medication therapy management.

Patients may access Monogram through their health plan. Monogram has tied-up with top insurance providers including Humana, Cigna, Point32Health and Centene, as well as risk bearing platforms including agilon, Advent and Banner Health.

The funding round was led by strategic investors CVS Health, Cigna Ventures, Humana, Memorial Hermann Health System, and SCAN, as well as financial investors TPG Capital, Frist Cressey Ventures, Heritage Group, Pura Vida Investments, and Norwest Venture Partners.

Mike Uchrin, CEO and Co-Founder of Monogram Health said in a statement: “Everything we do is designed to improve access to health care and health outcomes for polychronic patients by supporting their individual needs in their own homes, transforming the patient experience, especially for those who have been historically underserved.”

Founded in 2019, the company has raised $555 million so far.
