Faris Ghawi, Vytalize Health’s Co-Founder and CEO, emphasized the importance of protecting and rewarding independent primary care physicians, who have a significant impact on healthcare quality and spending.

Vytalize Health partners with over 2,500 leading providers to offer evidence-based recommendations at the point of care, making practices more efficient and effective. The platform incentivizes practices to follow recommendations and drive better health outcomes for patients.

Vytalize Health also operates a virtual and in-home clinic and has launched several initiatives, including the Priority Care program. This program uses data to identify patients with the greatest needs, extends practices’ reach to patients’ homes, and considers non-clinical factors in patients’ environment, taking a holistic approach to care.

By delivering the right data at the right time in the workflow to physicians and their staff, Vytalize Health enables its partners to focus on proactive, personalized medicine to improve the value of care and lower costs. The organization aims to build high-performing systems of care that advance quality, improve the patient experience, and are sustainable for providers and the healthcare system overall.

With the latest round, Vytalize has raised over $170 million.

In conclusion, Vytalize Health’s innovative approach to value-based care delivery is transforming the healthcare industry. With its cutting-edge platform, the organization is empowering primary care practices and improving the quality of care for seniors.
